3. MEDIA RELEASE FORM
3.1 PERMISSION
I grant Arts Unlimited Southwest permission to:
• Photograph
• Record video
• Capture audio
of myself and/or my child.
3.2 USAGE RIGHTS
I authorize AUSW to use media for:

• Marketing materials
• Educational content
3.3 NO COMPENSATION
I understand:
• No payment will be provided
• Media becomes property of AUSW

4. STUDENT / PARENT AGREEMENT

Students agree to:

4.2 DISCIPLINARY ACTION
AUSW may:
• Remove students from class
• Suspend or terminate enrollment
No refunds guaranteed in disciplinary cases.
4.3 ATTENDANCE POLICY
Students are expected to:
• Arrive on time
• Participate fully
4.4 PROPERTY RESPONSIBILITY
AUSW is not responsible for:
• Lost or stolen items
• Personal belongings

Users must not:
• Share accounts
• Upload harmful content
